Eligibility
30 hours per week on a regularly scheduled basis
Coverage begins first of the month following 60 days of working the required amount of hours
Open enrollment for the Providence medical plan is from January 1 – January 31 for a February 1 start date
Open enrollment for the Companion Life dental plan is from February 1 – February 28 for a March 1 start date
Employer pays 100% for employees on the medical and dental plan
Employees may enroll dependents on the plan with premium contributions
