Eligibility and Contribution
32 hours per week on a regularly scheduled basis
Coverage begins first of the month following 60 days of working the required amount of hours
Open enrollment is from March 1 through March 15 annual for a April 1 start date
Employer pays 100% of employee premium on the Connect 6900 plan
Employees may enroll dependents on the plan with premium contributions
