Eligibility and Contribution
The hourly requirement to be eligible is 32 hours per week
The eligibility waiting period is first of the month following date of hire, or date of hire if hired on the 1st of the month
Open enrollment is from December 1 through December 31 annual for January 1 start date
Employer pays 100% for employees only
Employees may enroll dependents on the plan with premium contributions
