- Out-of-network providers typically receive about 90% of their usual fees, though balance billing may apply.
- Some plans have waiting periods for basic or major services; however, certain carriers waive these periods or provide credit if members had prior dental coverage.
- Annual benefit maximums generally range from $1,000 up to $2,500.
- Orthodontic coverage may be available for children or adults.
- Pediatric preventive dental is required in many plans and can also be purchased as a stand-alone benefit.
