Eligibility
35 hours per week on a regularly scheduled basis
Coverage begins on the first of the month following date of hire
Open enrollment is February 1st through February 28th for a March 1st start date
Employer pays 75% for the Providence Connect 10000 Bronze Plan and 100% of the Standard dental plan
Employees may enroll dependents on the plan with premium contributions
The Providence Connect 10000 Bronze and Standard dental plan renew on March 1st each year
