Eligibility and Contribution
33 hours per week on a regularly scheduled basis
Coverage begins first of the month following 60 days of working the required amount of hours
Open enrollment is from June 1 – June 30 for a July 1 start date on the medical and dental plan
Employer pays 65% for employee only on the Providence Balance 4000 Silver
Employer pays 60% for employee only on the Companion dental plan
Employees may enroll dependents on the plan with premium contributions
