Eligibility and Contribution
The hourly requirement to be eligible is 32 hours per week

The eligibility waiting period is first of the month following 30 days
Open enrollment is from October 1 through October 31 annual for November 1 start date
Employer pays 100% for employees on the Companion Life Dental plan
Employees may enroll dependents on the plan with premium contributions
