Eligibility and Contribution
30 hours per week on a regularly scheduled basis
Coverage begins first of the month following 60 days of working the required amount of hours
Open enrollment is from December 1 to December 31 each year
Employer contribution is 100% of the Standard Bronze Employee Only premium, and 0% for Dependents
Employees may enroll dependents on the plan, and pay premiums with pre-tax payroll deductions, subject to plan eligibility
