Eligibility and Contributions
Employees must work 40 hours per week on a regularly scheduled basis
Coverage begins first of the month following 60 days of employment
Open enrollment is May 1st through May 31st for a June 1 start date
Employer pays 50% for employee only Medical premium on the lowest cost option
Employer pays 50% for employee only on the Choice Dental Plan
Employer pays 50% for employee only premium on Spectera Vision plan
