Eligibility
36 hours per week on a regularly scheduled basis
Coverage begins first on the first of the month following 60 days of working the required amount of hours
Open enrollment is August 1 – August 31 annually for a September 1 start date
Employer pays 80% of premium on the Providence Standard Gold Plan
Employer pays 100% of premium on the Providence Advantage Access Dental and Renaissance Vision Plans
Employer pays 0% for dependents
