Disabled American Veterans pays 100% for employees only on the Providence Standard Gold plan, and 0% for dependents
Disabled American Veterans pays 50% for employees only on the Companion Dental Plan, and 0% for dependents
Employees may enroll dependents on the plan with premium contributions
