Eligibility
28 hours per week on a regularly scheduled basis
Coverage begins first of the month following 60 days
Open enrollment is from December 1 through December 31 annually
Employer pays $400 for employees only on medical, 5% dental premium for employees and dependents, and 100% of employee only premium on Life and AD&D
