Eligibility and Contribution
32 hours per week on a regularly scheduled basis for medical, 24 hours per week on a regularly scheduled basis for dental
Coverage begins first of the month following 60 days of working the required amount of hours
Open enrollment is from September 1 to September 30, for an October 1 start date annually for Providence medical plan and November 1 to November 30 for a December 1 start date for Companion Dental plan
Employer pays 50% for employee only on the Providence Standard Bronze plan and Companion Dental plan
Employees may enroll dependents on the plan with premium contributions
