Eligibility and Contribution
24 hours per week on a regularly scheduled basis
Coverage begins on the first of the month following date of hire
Open enrollment is from October 1st through October 31st for a November 1st start date
Employer pays 50% for employee only on the Providence Connect 2500 Gold plan, and employees can buy up to Choice 2500 Gold with additional premium contributions
Employer pays 50% for employee on the Companion Life dental plan
Employees may enroll dependents on the plan with premium contributions
