Eligibility
30 hours per week on a regularly scheduled basis
Coverage begins on the 60th day of working the required amount of hours
Open enrollment for the Kaiser health plan is from April 1 – April 30 for a May 1 start date
Open enrollment for the Renaissance dental plan is from December 1 – December 31 for a January 1 start date
Employer pays 75% for employee only on the Kaiser Silver 3000/40 plan, employees can can choose a different plan with additional premium contribution
Employer does not contribute to the dental plan
Employees may enroll dependents on the plan with premium contributions
The Kaiser Health Plans renew on May 1st each year, and the Renaissance Dental plan renews on January 1st each year
